How to Become A Medical Marijuana Patient
1. Patients should obtain legitimate medical records or documentation from their primary care physician describing the diagnosis
2. Patients need a signed Physician’s Statement from a physician, currently licensed to practice medicine in Massachusetts Patients must be 18 years of age, and be a Massachusetts resident, with a valid proof of residency.
3. To qualify, a patient needs to be diagnosed by a physician as having one of the following debilitating medical conditions that is specifically identified in the law: cancer, glaucoma, positive status for human immunodeficiency virus (HIV) / acquired immune deficiency syndrome (AIDS), hepatitis C, amyotrophic lateral sclerosis (ALS), Crohn’s disease, Parkinson’s disease, multiple sclerosis, and other conditions as determined in writing by a qualifying patient’s physician.
4. Patients are required to register with the Massachusetts Department of Health and Human Services. Registration is mandatory.
5. The patient registry application fee is $50.
6. A qualifying patient may apply to the Department of Public Health of the Commonwealth of Massachusetts, by submitting:
(a) Written certification from a physician.
(b) An application, including:
i. Name, address unless homeless, and date of birth.
ii. Name, address and date of birth of the qualifying patient’s personal caregiver, if any.
